Need to modify an existed form, but with

I have 2 forms in total. But I need a form in which by clicking one link only that particular form loads next. Or by clicking the link it shows selective fields only. Form is partially completed in Indesign and need to modify a bit only. Form format I need is in PDF possible, but other options can be preferred.
