I usually work through a document twice, depending on its length. With long manuscripts I return it to the client for approval and to implement any suggestions before reviewing it a second time, including proofreading. For example, when working on a memoir I noticed that the client had skimmed over an important section and asked her to supply more information. I then slotted the additional material into the text.
I generally work either in Word using tracked changes or from a PDF in Editing or Suggesting mode. With a Word document I supply a clean copy with all changes implemented as well as the marked-up version.
